Palliative Care Early and Systematic (PaCES) Project

 

PaCES is a team of researchers and knowledge end-users working together to implement evidence-based practices for delivering early and systematic palliative care for Albertans living with serious illness. Their goal is to improve patient outcomes and decrease health system inefficiencies, by increasing the number of patients receiving earlier palliative care. Mike work with the PaCES team to create digital stories with their patient and family advisors. These stories are used in various educational and Quality Improvement contexts.
